I acknowledge and consent to the capture of photographs for promotional purposes by the Club, if you do not consent to photographs being taken of your child, please inform a staff member on the day. I understand that all children must be accompanied and supervised by a Club member or authorised guardian at all times while on Club premises or participating in Club activities.

I understand that I am required to collect my wristband prior to the day of the event and that I must be, or become, a Club member, as membership status will be verified upon collection at Member Services Workers Sports.
